Mitral valve stenosis and left ventricular hemodynamic alterations after mitral valve repair

چکیده
Background Mitral valve repair (MVR) shows clinical and economic superiority over mitral valve replacement in patients with dystrophic and rheumatic regurgitation, but outcomes are less certain in chronic ischemic mitral regurgitation (IMR). We hypothesize that MVR may have adverse effects on LV hemodynamics resulting from suboptimal annuloplasty geometry and post-repair mitral valve stenosis whose consequences are most acutely felt in IMR patients because of their impaired LV function. To test this hypothesis, we qualitatively described post-repair hemodynamics and measured LV inflow quantities (peak diastolic inflow velocity, transmitral pressure, and inflow angle) after MVR in sheep.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...

Background: Percutaneous Transeptal Mitral Commisurotomy (PTMC) has been used in treating mitral stenosis with variable success, its main complication being mitral incompetence. There is a need to define the subgroup of the patients who benefit mostly from the procedure. Methods: We studied 110 patients (age 17 to 60 years mean 33.2) with mitral stenosis. PTMC was performed though femoral vein....
متن کاملLong-term Results After Open Mitral Commissurotomy for a One-Month-Old Infant With Mitral Stenosis.
The strategy for an infant with congenital mitral stenosis should be determined by three important factors: left ventricular volume, the degree of the systemic outflow tract obstruction, and the type of mitral valve dysfunction. A successful staged biventricular repair in early infancy for a patient who had congenital mitral stenosis with short chordae, hypoplastic left ventricle and coarctatio...

A 47 year-old female died after mitral valve replacement. Post-cardiopulmonary bypass right ventricle was not able to pump despite good left ventricular contractility. At microscopic examination, diffuse right ventricular fatty infiltration was found. We found no previous report of this pathology in patients with mitral stenosis
متن کاملRight sided heart evaluation after successful mitral valve replacement.
Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...
